• Commercial Drivers License
• Medical Card
• Log(s) seven days previous to start date
• Social Security, Visa, Work Permit
• Articles of Incorporation, if incorporated
• Tractor Rgistration, Apportionment Identification Card
• Current FHWA Annual Inspection with contact info of shop providing service
• 10 Year work history (no gaps) with contact phone/fax numbers for last 3 years
• 2290 and Title (Front and Back)
• W-9

Optional Documents

• Recent maintenance records
• TWIC Card
• Passport or Passport Card
• Checking/Savings info if opting for Direct Deposit (voided check or deposit slip)
